Annually, active employees can make new selections for their health benefit needs.  The window for making these changes is open from May 20 thru May 31, 2024.

Double click on the election form that matches your situation, then save to your device to enable the fillable format.  Make your selections, sign the form, save your filled in document for your records and send the completed form to the Business Department.  You may submit this form as an attachment to an email or print and send through the District mail. Only those employees that have a spouse that is also covered by SISC through their employer should select the Dual Insured 75% form.
